POSITION SUMMARY

The Home Health and Hospice Aide provides personal care and related services to assigned patients under the direction and supervision of the patient's case manager. The aide is responsible for documenting all care provided and communicating regularly to other members of the patient care team regarding the patient's status.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Follows the Aide Activity Plan safely and accurately as delegated by the Case Manager in accordance with ADORAY Home Health and Hospice policies and procedures. Activities may include but are not limited to: bathing, skin care, hair care, nail care, transfers, assistance with ambulation, toileting, self-administration of medications, and light housekeeping.
• Assists with assigned nursing duties when instructed and supervised by an RN, such as simple dressing changes and indwelling catheter care.
• Assists, as assigned, with prescribed exercises which patients and/or aide has been taught by an appropriate health professional.
• Notifies the case manager or clinical supervisor of any deviations from the Activity Plan.
• Teaches safety to patients and/or care givers as appropriate.
• Complies with Universal precautions, standard infection control protocols and general safety policies and procedures.
• Reports changes in patient's condition, including pain control to case manager or clinical supervisor within one hour of assessment.
• Accurately completes appropriate clinical records related to assigned duties and observations of patients within 48 hours of the provision of care.
• Works effectively as a member of the interdisciplinary team and contributes to plan of treatment based on knowledge and experience.
• Maintains own work schedule and completes work schedule for upcoming week in Cerner no later than 1600 on Thursday.
• Any cancellation of a visit to a patient must be communicated by phone to the Home Health Scheduler or Hospice Coordinator who will schedule another staff member ASAP.
• Participates in in-service education programs offered by Adoray Home Health and Hospice at least six times per year.

Requirements

Minimum Qualifications

• Satisfactory completion of training program or competency test for Home Health aide which meets the requirements of the State of Wisconsin Department of Health as well as Medicare (CNA).
• Home health and/or hospice experience preferred.
• Current Wisconsin Driver's license.
• Name is listed on the Wisconsin and Minnesota registries as current and eligible to provide 
• Home health and hospice care.
• Excellent communication skills.
• Evidence of attendance of hospice training class or willingness to attend.
• Believes in and has a working knowledge of the hospice philosophy of care.
• Commits to work at least 18 hours per month to maintain competency.
• Basic Computer skills.
