Job Summary: This position works collaboratively with the SJCS-TN management team to provide and/or integrate optimal health care and safety into programs and activities for individuals served in compliance with licensure and other regulatory standards. The nursing process is used to train, assess, make recommendations about and evaluate care of assigned individuals.
Essential Functions:
  1. Provides maintenance of monthly medical notes, nursing assessments and other documentation related to medication use, general health status, and emergency procedures for assigned caseload.
  2. Consults with physicians, pharmacies and other medical personnel on individual needs and relays necessary treatment information to staff to ensure follow-up on such needs.
  3. Conducts monthly observations of individuals identified with acute/significant medical needs and follows for Medical Services review, providing documentation as applicable/required.
  4. Provides consultation and practical advice to management and staff regarding routine medical issues and concerns.
  5. Provides state approved Medication Administration training for staff as required by TN state law.
  6. Assesses staff needs regarding medication administration protocols and develops programs to minimize staff medication errors.
  7. Assists management in evaluating staff compliance with TN State medication management regulations according to physician's instruction and state law.
  8. Performs other job related duties as assigned.
Secondary Functions:
  1. Serves as member of TN management team. Represents SJCS in external disability related medical associations as requested
  2. Assists in the annual development and quarterly implementation of SJCS Medical Services Committee competency-based training as well as other training as identified.
  3. Serves as member of the Medication Administration Review Committee (MARC) for assigned region.
  4. Performs other job related duties as assigned.
Required Knowledge, Skills, and Abilities:
  1. Ability to interact with individuals with disabilities and staff supporting these individuals
  2. Knowledge of federal regulations applicable to nursing and health services for ICFMRs ;
  3. Skilled in nursing practices and/or protocols necessary to ensure health and safety of consumers.
Physical and Environmental Conditions:
Work environment involves everyday risks or discomforts which require normal safety precautions typical of such places as offices, e.g., use of safe work place practices with office equipment, and/or avoidance of trips and falls.
Home environment involves everyday risks or conditions that require normal safety precautions such as stairs, rugs, appliances etc. and requires the avoidance of trips and falls. Additionally, medical work with individuals involves knowledge of medical protocols, exposure to human behavior, and other safety precautions. Proper precautions must be taken when assessing and/or interacting with individuals served with regard to lifting, transferring, turning, positioning, administering medications, etc.

Qualifications
  1. Current Registered Nurse license in Tennessee required; Certified Developmental Disability Nurse (CDDN) certification is a plus.
  2. Requires 1 year experience working with individuals with intellectual disabilities
  3. Requires 2 years of experience working as a Community Based Nurse.
  4. Experience with or working knowledge of Tennessee regulations preferred.
  5. Supervisory experience preferred
  6. Must meet SJCS and funding jurisdiction current requirements for criminal background/records check.
  7. Valid health certificate to meet current funding jurisdiction requirements.
  8. Valid state-issued driver's license with current driving record in good standing required.
