Description:
Provides clinical case management functions via a collaborative process that transitions patients to optimal clinical outcomes while assuring safe, cost-effective care. Demonstrates a daily commitment to the department's value of teamwork. This position is an integral member of a multidisciplinary community network, provides administrative coordination and is the point of contact for individuals with opioid use disorders in receiving medication assisted treatment, substance use resources, behavioral health, and support services.
Requirements:
• MSW (Masters Social Work)
•1 - 2years Acute Social Worker Experience. (no exceptions & ideally within discharge planning)
•Licensed Social Worker -- LSWAIC within the state of WA
Preferred
•ACM/CCM certification (can have upon starting)
•BLS AHA Certification
Department: Discharge Planning
Shift:Shift: 5x8s Days (07:00 - 15:30)
