Achieve Rehab and Nursing Facility is a prestigious and well-established Skilled Nursing Facility nestled in the beautiful Catskill mountains. This newly-renovated Facility is the host of 140 resident beds; including modern, private suites for subacute rehabilitation services.

Achieve RNF is presently seeking a passionate and dedicated Director of Nursing Services to join our growing team of healthcare professionals.

The Director of Nursing Services primary responsibility is to plan, organize, develop, and direct the overall operation of the Nursing Services Department in accordance with current federal, state, and local standards, guidelines, and regulations that govern our Facility and as may be directed by the Administrator or the Medical Director to ensure that the highest degree of quality care is maintained at all times.

Director of Nursing Services candidates MUST:

• Be familiar with PDPM, CMS Final Rule
• Possess a current, unencumbered, active license to practice as a Registered Nurse in New York State.
• Have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care Facility, or other related health care Facility.
• Have, as a minimum six (6) months experience in rehabilitative and restorative nursing practices.
• Be a supportive team member, contribute to and be an example of team work and team concept.
• Be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.

Responsibilities for the Director of Nursing include:

·General oversight of nursing personnel by making rounds frequently, ensuring the department is adhering to current Federal and State regulations

·Making recommendations on staffing ratios, assignments to ensure the needs of the residents are being met to the highest standard

• Following up on grievances
• Discipline and performance appraisals for nursing personnel, as necessary
• Maintaining a strong rapport with all inter-departmental team members including physicians and vendors
• Participate in committees including Infection Control, Quality Assurance, Utilization Review, clinical meetings, etc.
