Are you a Registered Nurse (RN) looking for a change of pace? Accurate Home Care and its Home Health service line is growing and adding an RN Case Manager to its team.

What’s in it for me?
- Hours/days: Monday – Friday, hours vary based on client base needs; one weekend a month
- Salary: $72,000- $80,000/ year paid mileage

What will I be doing?
- Complete patient visits as needed under the direction of the Director of Home Health.
- Participates in the implementation and oversight of nursing and home health aide services.
- Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge.
Requirements
What skills do I need to bring?
- Must be licensed as a Registered Nurse (RN) in Minnesota
- Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies
- Must show proof of COVID-19 vaccination or medical or religious exemption
- Must be able to lift, turn, position, and transfer up to 50lbs
- Must be fluent in the English language
- Valid U.S. driver’s license with minimal moving violations and access to a reliable personal vehicle
