Access TLC Health Care is Hiring a Care Coordinator Near San Fernando, CA

Access TLC is currently seeking for a Full-time Care Coordinators to join our team. The Care Coordinator provides in-office case management and regular field visits to patients.

Access TLC support patients by arranging for community-based and other services that allow recipients to experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind and a genuine team player.

Position Responsibilities:

  • Conducting assessments, developing care plans, arranging for and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data.
  • Makes visits to patients to establish ongoing eligibility for the program, to monitor the safety and services identified in the Individual Service Plan and address the concerns of the patient.

Requirements Include:

  • Must have either a Bachelor or Master Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, Health or Human Services or related field
  • Must have a clean California Driver’s License with a vehicle that is insured in accordance with state/or organization requirements
  • One year of related work experience. Intern or volunteer experience is also applicable.
  • Bi-Lingual is a plus!

Job Type: Full-time Monday through Friday 8:30 am – 5:30 pm

Rate of Pay: $22.00 /hour
