Description:

Plans, organizes, develops and directs overall operation of the nursing services of the skilled nursing unit in accordance with current federal, state and local standards, guidelines and regulations that govern the facility and as may be directed by the administrator and the medical director, to ensure that the highest degree of quality care is maintained at all times.

Responsibilities:

Includes the following. Other duties may be assigned.

Plans, develops, organizes, implements, evaluates and directs the skilled nursing department, as well as its programs and activities, in accordance with current regulations.

Maintains the guidelines that govern the long-term care of the facility.

Develops, maintains and periodically updates the written policies and procedures that govern the day-to-day functions of the nursing department.

Develops, maintains and periodically updates the nursing procedure manual (s) and nursing service objectives and philosophy.

Assists the quality assurance committee in developing and implementing appropriate plans of action to correct identified deficiencies.

Develops methods for coordination of nursing services with other resident services to ensure the continuity of the resident's total regimen of care.

Develops, maintains and periodically updates written job descriptions for each level of nursing personnel.

Performs administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.

Maintains a reference library of written nursing materials (federal, state regulations, PDR's, etc.) that will assist the department in meeting the day-to-day needs of the resident.

Develops, implements and periodically updates written procedures for discharge planning.

Participates in surveys (inspections) made by authorized government agencies.

Makes oral and written reports/recommendations to the administrator as necessary/required over concerns of the operation of the department.

Serves on, participates in and attends various committees of the facility (patient care, quality assurance, infection control, etc.) as required and as appointed by the administrator.

Provides written/oral reports of nursing programs to committees as required/appropriate.

Schedules, attends and participates in resident care plan meetings.

Evaluates and implements recommendations from established committees and consultants as they may pertain to nursing services.

Interviews and selects residents for admission to the facility, including the MI/MR pre/annual screening.

Provides the administrator with information relative to the nursing needs of the residents and the nursing department's ability to meet those needs.

Informs personnel of new admissions, time of arrival, room assignments, etc.

Ensures that rooms are ready for new admission and greets the newly admitted residents/families upon admission, escorts to room if necessary.

Reviews nurse's notes to ensure that they are informative and descriptive of nursing care being provided and that they reflect the resident's response to the care.

Makes rounds with physicians as necessary, encouraging attending physicians to record and sign progress notes, physician orders, et., on a timely basis and in accordance with current regulations.

Schedules daily rounds to observe residents and to determine if nursing needs are bring met.

Monitors medication and passes treatment schedules to assure medications are being administrated as ordered and that treatments are provided as scheduled.

Provides direct nursing care as necessary and authorizes the use of restraints, physical and chemical, when necessary in accordance with established facility policies and procedures.

Develops preliminary and comprehensive assessments of the nursing needs of each resident.

Develops a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, the time frame and which professional services are responsible for each element of care.

Encourages the resident and/or family member to participate in scheduling of care plans and assessments to be discussed at each care plan meeting.

Reviews nurse's notes to determine of the care plan is being followed and reviews and revises care plans and assessments as necessary, but at least quarterly.

Maintains confidentiality of all resident care information.

Monitors nursing care to assure that all residents are treated fairly and with kindness, dignity and respect.

Ensures that all personnel knock before entering the resident's room.

Ensures that all nursing personnel are knowledgeable of the resident's rights and responsibilities.

Reviews complaints and grievances made by the resident/family member (s) and make a written/oral report to the administrator indicating what action (s) were taken to resolve the complaint or grievance.

Maintains a written record of the resident/family member (s) complaint/grievance that indicates the action being taken to resolve the complaint or grievance and the current status.

Determines staffing needs of the nursing department with are necessary to meet the total nursing needs of the residents and assigns sufficient professional and non-professional staff to ensure that the quality of care is maintained to recruit and select nursing service personnel.

Develops work assignments and schedules duty hours.

Delegates administrative authority, responsibility and accountability necessary to perform assigned duties.

Develops, maintains and periodically updates written procedures for ensuring the professional nursing personnel have valid and current licenses as required by the state.

Makes daily rounds of the nursing department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards.

Reviews complaints/grievances made/filed by department personnel.

Makes 90 day and annual evaluations of work performance of department personnel, reviewing with administrator.

Ensures that disciplinary action is administered and without regard to race, color, creed, nationality, origin, age, sex, religion, handicap or marital status.

Monitors nursing personnel to assure they are following established safety regulations in the use of equipment and supplies.

Ensures that personnel work areas, resident care rooms, and community rooms are maintained in a clean, safe and sanitary manner.

Assists the infection control committee in development, implementation and revision of written aspect and isolation techniques.

Develops, implements and maintains a program for monitoring communicable and/or infectious diseases among residents and personnel.

Ensures that nursing personnel follow established infection control procedures when isolation precautions become necessary.

Develops, implements and maintains a procedure for reporting hazardous conditions or equipment.

Identifies, evaluates and classifies routine job-related functions to ensure that tasks in when potential exposure to blood/body fluids are properly identified and recorded.

Assures that all personnel performing tasks that involve potential exposure to blood/body fluid participate in in-service training prior to performing such tasks.

Recommends to the administrator the equipment and supply needs of the department.

Assures that a stock level of medication, medical supplies, equipment, etc., is maintained on premises at all times to adequately meet the needs of the resident.

Develops and implements procedures for the safe operation of all nursing service equipment.

Ensures that only trained and authorized personnel operate the department's equipment.

Tasks assigned to this position may involve exposure to blood/body fluids. You may be exposed to the HIV and Hepatitis B viruses. The facility will make available to the employee, free of charge, the Hepatitis B vaccine.

Minimum Job Requirements

Must posses, as a minimum, a nursing degree from an accredited college or university.

Must have as a minimum, three years experience as a supervisor in a hospital, long-term care facility, or other related health care facility.

Must have, as a minimum, six months training experience in a rehabilitative and restorative nursing practices.

Must posses a current, unencumbered license to practice as a registered nurse in this state and must have a current certification for CPR.

Must posses the ability to understand and follow verbal and written instructions.

Must be able to relay information concerning a resident's condition.

Must posses the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies, procedures, regulations and guidelines which pertain to nursing service in the long-term setting.

Must posses the ability to make independent decisions when circumstances warrant such action.

Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.

Must be able to relate to and with the ill, disabled, elderly, emotionally upset and at times, hostile persons within the facility.

Must have patience, tact, a cheerful disposition and enthusiasm as well as the willingness to handle difficult residents and family members.

Stands and walks continuously up to 8 hours per shift; bending when monitoring resident care conferences, committee meetings, scheduling, documenting health records, writing policies/procedures, speaking with residents/family members, physicians/consultants.

Squats occasionally up to 2 hours per shift; will squat when monitoring resident care, staff procedures, inspecting nursing unit and when providing direct resident care.

Reaches occasionally up to 3 hours per shift; will reach when monitoring resident care, staff procedures, inspecting nursing unit and when providing direct resident care, documenting in resident's health records or scheduling.

Pushes/pulls occasionally up to 2 hours per shift; will push wheelchairs/Geri-chairs in which residents are seated weighing as much as 200 pounds, treatment/medication carts, food carts and medical record carts.

Lifts/carries occasionally up to 3 hours per shift; will lift/carry equipment and supplies, trays containing resident meals and resident health records.

Assists in the evacuation of residents weighing as much as 200 pounds.

Job Type: Full-time

Pay: $151,977.00 - $199,509.00 per year
