Thank you for considering a career at Mercy Health Position Summary Responsible for coordination of an interdisciplinary team approach in the provision of skilled nursing assessments, planning and care in order to maximize the comfort and health of patients and families consistent with the policies and procedures of Lourdes Homecare/Hospice. Develops a holistic multidisciplinary plan of care utilizing the job functions within each of the five core components of case management. Interacts with the care management team and physicians to determine appropriate home care/ hospice service for patients/clients based on regulatory guidelines for each line of service Minimum Required Qualifications Education: Graduate of accredited school of nursing Licensure: Licensed as a registered nurse in KY and Illinois. Current BLS certification required Certification(s): Experience: one year of Homecare Hospice exp as part of a care team preferred. Preferred Qualifications Education: Licensure:Certified Case Manager preferred Certification(s): Experience: two years of acute nursing experience med-surg, OB psych,peds etc. Other Knowledge, Skills and Abilities Valid drivers license and auto insurance required.
