Thank you for considering a career at Mercy Health PRN SUMMARY OF JOB RESPONSIBILITIES: Compounds and dispenses medication pursuant to a physician order. Monitors drug therapy for appropriateness of drug, dose, route and duration of therapy. Screens for drug interactions, incompatibilities and contraindications. Recognizes potential adverse drug effects and intervenes as appropriate. Provides drug information to physicians, nurses, allied health professionals and patients. Participates in Interdisciplinary Team Conferences and other patient-care activities. Cares for all age group patients (birth and above). QUALIFICATIONS: EDUCATION: · B.S. Pharmacy degree from accredited College of Pharmacy. · Licensed as a Registered Pharmacist by the Ohio State Board of Pharmacy (or must be Ohio-licensed within 90 days of employment). · Must provide evidence of meeting state-mandated continuing education requirements upon request. EXPERIENCE: · For third-shift positions, at least one year experience as a staff pharmacist in a hospital pharmacy is required. · For all other shifts, previous hospital pharmacy experience is highly desirable. · Previous pharmacy computer experience desirable. KNOWLEDGE AND SKILLS: Knowledge and skill required to perform the job responsibilities for both technical and dimensional content. Has broad working knowledge of the practice of Pharmacy. Analyzes and interprets complicated information. Determines a course of action based on general guidelines. May modify existing forms, procedures or methods. May use highly sophisticated equipment requiring extensive training. Individual must remain current in his/her knowledge of drugs, therapeutics and drug delivery systems. · PROBLEM SOLVING/DECISION MAKING: The degree to which judgment and analysis must be exercised in planning, investigating issues, and evaluating alternative solutions to the challenges posed by the job. Deals with the nature of the decisions, the variety of problems, and how decisions are made in context of the typical requirements of the job. Makes decisions based on an analysis of facts and general principles. Receives guidance through broad policy only and has varied courses of action available. Requires risk-taking and judgment to adapt existing philosophy or practices. Ability to solve problems and/or decisions made frequently that have direct impact on patient care and/or an individual patient’s course of treatment. Considerable professional judgment required. · SCOPE OF IMPACT/ACCOUNTABILITY: The potential impact of actions on results of HM Health Services and the job’s accountability for achieving results. Impacts multiple work areas on a day-to-day basis. Provides information regularly to decision-making process. Has little or no responsibility for committing resources (financial or staff) in work area, but performs analyses and makes recommendations. · INTERPERSONAL COMMUNICATION: The nature and frequency of interpersonal human relations skills required by the job. The extent to which the job requires cooperation and tact in meeting or influencing others by phone, correspondence, or personal contact. Provides team leadership by assisting and training others. Responds to non-routine and detailed inquiries from others who may not be knowledgeable in subject area.
