Position Summary
This role is performed face-to-face/field-based and requires those in the position to travel at least 50% of the time outside of the home/office to various member homes, various facilities (such as nursing homes, rehab centers, etc.), or community meetings on a defined schedule, adhering to the defined metrics, as required by the business. Nurse Case Manager is responsible for telephonically and/or face to face assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.

Required Qualifications:

  • 3 years clinical practice experience
  • Minimum 2 years of Case Management in a Managed Care environment
  • Active unrestricted NC Compact RN Licensure
  • This role is performed face-to-face/field-based and requires those in the position to travel at least 50% of the time outside of the home/office.
  • Travel will be within 50 miles of Chapel Hill, NC (27514) or Concord, NC (28025)

Preferred Qualifications:

  • Previous Case Management or Managed Care experience
  • Previous experience with Home Health
  • Typical office working environment with productivity and quality expectations
  • Work requires the ability to perform close inspection of hand written and computer generated documents as well as a PC monitor.
  • Sedentary work involving periods of sitting, talking, listening. Work requires sitting for extended periods, talking on the telephone and typing on the computer.
  • Ability to multitask, prioritize and effectively adapt to a fast paced changing environment
  • Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding
  • Effective communication skills, both verbal and written.

Education:

  • Associates Degree or Nursing Diploma Required
  • Bachelors Degree Preferred

Pay Range

The typical pay range for this role is:

$54,095.60 - $116,760.80

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
