Description of Duties and Responsibilities

The Follow Up Support Worker (FSW) works intensively with a specific caseload of clients coming from outreach and/or encampment teams within the city. The FSW supports individuals with complex needs, who may require multiple visits a week and higher-level support systems in order to live independently in market housing. A key component of the Follow-Up Support Worker position is that it is pre-emptive, identifying problems before they come to crisis, saving emergency resources and maintaining a stable housing environment for the tenant, landlord and surrounding community. In addition, the Follow-Up Support Worker will:

  • Provide client assessment, life-skills training
  • Maintain landlord relationships and assist with tenant mediation
  • Case co-ordination and provide crisis intervention.
  • Interact with clients in a caring, non-judgemental manner
  • Monitor clients on a daily basis and communicates concerns and recommendations to the Program Manager.
  • Record client information in ETO and update daily
  • Attends training as necessary
  • Adhere to the principals of Housing First as described in the Housing First Procedures manual
  • Attend general staff meetings
  • Any other related task or responsibility assigned by the Program Manager.

Position Qualifications

  • Demonstrated level of literacy and communication skills sufficient for job responsibilities
  • An understanding that the assigned caseloads will require intensive supports throughout the week, as well as prioritizing administrative duties and contractual obligations
  • An open, accepting attitude
  • Experience in working in the addictions field
  • Experience working with individuals with complex needs (i.e., dual diagnoses, chronic homelessness, physical health concerns, etc)
  • Has established networks within the community
  • Diploma or degree in a human services field and 1 to 2 years’ experience working with a marginalized population.
  • Current Vulnerable Sector Police Information Check
  • Valid Driver’s license
  • Own vehicle with $2 million liability insurance and may transport clients as needed

Hours of Work: Monday to Friday, 8 hour shifts
