Description of Duties and Responsibilities
The Follow Up Support Worker (FSW) works intensively with a specific caseload of clients coming from outreach and/or encampment teams within the city. The FSW supports individuals with complex needs, who may require multiple visits a week and higher-level support systems in order to live independently in market housing. A key component of the Follow-Up Support Worker position is that it is pre-emptive, identifying problems before they come to crisis, saving emergency resources and maintaining a stable housing environment for the tenant, landlord and surrounding community.
