About The Role:
· Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health and diabetes management
· Clinical delivery and supervision of non-clinical team members in a community setting
· Intake processing, clinical care planning and reporting
· Conducting client care assessments with a focus on Consumer Directed Care
· Coordinate the provision of quality person-centered care according to the client's needs, service/care plans and available funding according to the client's budget
· General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation
· On-call roster
Qualifications
Essential
· Qualified Registered Nurse
· Current AHPRA registered
· First Aid & CPR
· Australian National Police check
· Professional Indemnity Insurance
· Experienced in assessing and planning care needs within the community
· Valid Driver's License and reliable presentable car
· A smart mobile phone (iPhone / android)
Preferred Skills
· A passion for high quality client-focused care in a community setting
· Cert IV in Training and Assessment - highly desirable
· Exceptional communication skills
· Able to work autonomously or within a team environment
· Excellent time management and organisation skills
